Management of a Heavily Draining State IV Sacral Ulcer:"An 87 year old female patient with a history of progressive senile dementia and congestive heart failure was admitted to our skilled nursing facility with a sacral wound. Clinical assessment revealed the wound was a stage IV pressure ulcer, due primarily to her limited mobility and incontinence of bowel. Upon admission, the wound measured 5.5cm x 8cm, and was 2cm deep, with undermining of 1cm laterally along one complete edge. Approximately 60% of the wound surface was covered with hard, necrotic tissue, and 25% was covered with yellow slough. Drainage from the wound was heavy, dark and thick, with a foul odor." ******************************************************
Management of Chronic Venous Stasis Ulcers:"A 90 year old female presented on 3-24-97 with chronic ulcers of the right lower extremity, medial and lateral, measuring 8.0cm x 15.0cm and 7.0cm x 5.0cm respectively. The ulcers were draining heavily and were very painful. The patient suffers from senile dementia, peripheral vascular disease and cellulitis, but otherwise is quite healthy. A number of treatment modalities, including compression therapy, and alginate, foam, hydrogel and wet-to-dry dressings were used over the ensuing two months without significant results. Consultations with Vascular and Plastic Surgeons confirmed that the patient had no arterial insufficiencies and was not a candidate for skin grafting. Amputation was the recommended course of action." ******************************************************
Management of a Necrotic Trans-Metatarsal Amputation Incision:"A 74-year-old male patient was admitted to home health following simultaneous right trans-metatarsal amputation and bilateral fem-pop bypass surgery. Within two weeks, the amputation incision became necrotic. The patient's history included congestive heart failure, long term diabetes and kidney disease. Previous vascular related surgeries included coronary artery bypass, and an amputation of a toe on the left foot. The patient's vascular surgeon was certain that a right BKA was most likely the only resolution for this patient." ******************************************************
Control of Bacterial Bioburden in a Stage IV Pressure Ulcer Using AcryDerm® Silver Antimicrobial Dressing:"CS is a 74 year old female with cerebral palsy, malnutrition and severe flexion/abduction contractures; estimated body weight is approximately 65-75lbs. She has a history of recurrent pressure ulcers and the most recent to develop is a stage IV of the right hip. This bedbound patient lives in a single family home with her older sister as primary caregiver. Due to CS’s level of care and her sister’s poor health, skilled nursing and home health aide services are being delivered. The patient is alert and able to express her needs even though she is essentially non-verbal. She leaves no doubt that she insists that she be positioned on the affected side. CS is not a candidate for tube feeding as per family wishes. Additionally, the patient is on "comfort measures" and as such, may not be transported from the home for any type of evaluation or treatment." ******************************************************
